Duringthecommunityassessmentthat tookplace,itwasnotedthatmanyAfrican American elderlypeopleare sufferingfrom type-2 diabetes. Themostcriticalthingabout thisis thattheyare not wellacquaintedon howto reducethechancesof thediseasethusitbecomesworse.Themajorcausesof thisdiseaseare pooreatinghabitsandlackof physicalexercise(CDC, 2011). Asa communityhealthcareprovider, I will stepintotheprogramto undertakevariousroles.

First,I will be creatingawarenessto thepeopleabout thedangersof poordietary tendenciesthatpropagatesthedisease.In doingthis,I will establishthe wayelderlypeopleshould feedto avertthedangersof thisdisease.Researchshowsthattheprimarycausesifdiabetesamong thesepeopleis poordiet(Imperatore et al. 2012). Secondly,I will actas an advocate towards thepromotionandadoptiontherightsupportinfrastructureto supportthehealthof theelderlypeople.

Thediabetic peopleneedplacesto exerciseandforsocialgatherings.Thehealthcarecentersfullyequippedto dealwith diabetes will haveto be provided(Cheng, Migliano &amp Zimmet, 2012).To createsocialchangein thecommunity,I will helptheelderlypeopleto adoptbehaviorchangein their dietandexercise.Theelderlyrequiresupportuntil thesafebehaviorbecomespartof their life(Nolan et al., 2011).

Thereareotherhealthcareproviders whofocuson type-2 diabetic people.I will createrapportwith them through theorganizationof outreach activitiesandothermedicalservicecamps.Workingalong thesameprincipleswill strengthenour bond.Of muchimportancewill be to createrapportwith thecommunitysothatall theeffortswill be compliantwith theculture.

Thepopulation-based nursingcareprojectwill be successfulbecauseitwill be a combinedeffortof thecommunityandhealthprofessionals.Communityparticipationwill be theprimaryinputthatthewayto smoothen thewayfora socialchange.
